The mission of the Akron Children’s Health Collaborative (ACHC) is to improve child health through family-centered care coordination, an engaged network of providers and a unified focus on quality, access and health equity.
Using this innovative model, ACHC is contracting with managed care companies to assume responsibility for care management and the cost of medical care for children across a Northeast Ohio service area. By standardizing best practices and implementing evidence-based care, consistent, high quality care will be delivered to all patients. As we deliver exceptional patient outcomes and high value care, the cost savings will be invested in children and communities.
A key component of ACHC is the integrated network of providers focused on child health. This includes pediatric and family medicine providers, pediatric subspecialists, behavioral health providers, women’s care providers, Clinically Integrated Networks (CINs) and Federally Qualified Health Centers (FQHCs).
