Objectives (Educational Content) :
1: Improve understanding of Population Health principles in care of patients at Akron Children's. 2. Identify opportunities in one's own practice to carry forward Population Health work. 3: Integrate Health Equity work for the populations we serve.
Target Audience:
General pediatricians, family physicians, nurse practitioners, physician assistants, social workers, psychologists, and nurses.
Identified Gap:
As we continue to serve the children who come to seek care at Akron Children's Hospital, we need to understand the role(s) Population Health principles play in this goal and utilize it to increase the quality of care provided to our patients and achieve superior outcomes.
