Objectives (Educational Content) :

1. Compare different health service approaches to multi-disciplinary care for children with medical complexity. 2. Explain the continuum of care integration, including blurred lines of clinical roles and responsibilities. 3. Illustrate how multi-disciplinary clinics advance shared decision making and patient/family experience for children with medical complexity.

Target Audience:

General pediatricians, family physicians, nurse practitioners, physician assistants, social workers, psychologists, and nurses.

There is a wealth of healthcare information that can be organized, sorted and leveraged to increase the quality of care we provide to our pediatric patients.
